One strong emotional response, difficult conversation, or request for reassurance may be ordinary. The structure begins when intensity, guilt, distress, concern, or reaction is repeatedly activated because that emotional energy preserves attention, engagement, or regulation. The emotion does not merely accompany the exchange; it helps keep the exchange alive.
